The Referral Process
- A request is made for a placement by completing and sending us a service user request form.
- Our placement services manager will check for providers who could meet the needs of the service user referred. If a vacancy is available the referrral will be allocated to a placement officer.
- The placement officer will discuss potential vacancies with you and may request more information.
- The placement officer will discuss the referral with a provider(s) who may be suitable and if a potential match has been found will discuss this with you.
- An introductory process will be agreed which may involve day visits, overnight visit(s) and/or a weekend visit.
- A decision is made by the service user and the provider separately about the match.
- If the match is felt to be desirable by all then the placement will be agreed and practical arrangements made.
Please Note:
The referal/matching process will take some time. We cannot accept emergency placements. If we do not have a suitable vacancy, your request can be put on our waiting list and will be regularly checked.
