SWAPS provides adult placements in the South West of England. SWAPS is a registered charity and a not for profit company. It currently operates within the geographical areas of Devon and Cornwall.
SWAPS makes and supports family based placements for vulnerable and disabled people over the age of 18 who may have a learning disability, mental health problems, physical disabilities and for older people.
Placement providers are individuals or families who are able to provide care and accommodation in their own homes for a vulnerable or disabled adult. Placement providers would usually support one service user but can provide up to a maximum of three places in their home.
Every provider who is part of SWAPS is supported by a SWAPS adult placement officer, who will usually cover a defined geographical area. The placement officer is responsible for managing the referral and placement process. They also assess, monitor and review providers to ensure that the scheme meets the requirements of the National Minimum Standards for Adult Placement Schemes and support providers through regular telephone and face to face contact, training and meetings.
SWAPS offers support and guidance to the placement provider, to ensure that the service user is receiving an appropriate level of person centred care and support in the family home. Adult Social Care departments retain care management responsibility for the service user where they have made the referral and will continue to review the service user's needs on a regular basis and manage aspects of their care and support that are not part of the placement.
