Long term placements
The opportunity to live with a family on a longer term basis, to maintain and expand independence and choice, enabling people to make full use of their local community resources.
Short breaks
Regular or one off short breaks that are fun and enjoyable for the service user and can offer a family carer a much needed respite break, ranging from a day time break, to long weekends right up to 28 day stays.
Dementia services
Specific short break services for people with dementia and their carers, with the benefit of the familiar domestic setting and the continuity offered by our providers.
Shared support and care arrangements
We can offer a service where a person may need accommodation and support for part of the year, or where a long handover/transitions into our service is required.
Transition services
We can work with other agencies to plan a transition into our service for a young person approaching adulthood, either by assessing and approving their current foster carers to become shared lives providers, or by finding a new match with one of our existing providers.
Intermediate “moving on” services
Shorter full time stays with a provider, specifically to gain daily living skills before a planned move to live independently in the community. Our providers can then go on to provide outreach support and opportunities to continue to share in family life.
