We are pleased that you are interested in SWAPS and Shared Lives services.
Referrals can be made by care managers in social services, health services and other care agencies on behalf of agencies. Referrals can also be made by service users, their family or representative.If you wish to make a referral please complete a Service User Request From and attach the requested supporting information before returning it to SWAPS.

The Referral Process:
- A request is made for a Shared Lives service by completing and sending us a service user request form
- One of our services managers will check for potential providers who could meet the needs of the service user. If a vacancy is available the referral will be allocated to a Shared Lives Coordinator (SLC).
- The SLC will discuss potential vacancies with you and may request more information.
- The SLC will discuss the referral with a provider(s) who may be suitable and if a potential match has been found will discuss this with you.
- An introductory process will be agreed which may involve day visits, overnight visit(s) and/or a weekend visit.
- A decision is made by the service user and the provider separately about the match If the match is felt to be desirable by all, then the placement will be agreed and practical arrangements made.
Please note: The referral/matching process will take some time. We cannot accept emergency placements. If we do not have a suitable vacancy, your request can be put on our waiting list and will be checked regularly."
